Return Merchandise Authorization - RMA

Please take a minute to fill out the following information so that we can quickly and efficiently process your return. Once you fill out and submit this form, we will email you an RA Number so that you can send your product back to us.

• You must call us first to obtain a verbal authorization code. 305-823-6991
• Forms filled out without the proper verbal code will be automatically rejected
• You must return your product with the provided RA # clearly on the outside of the box
• Products returned without the RA # on the outside of the box will be rejected by our receiving department and delay the returns process.
